Abstract

U današnje vrijeme bolni sindromi kralježnice su vrlo česti, stoga je važno imati dobru dijagnostiku stanja. Pojavljuje se veliki broj pojedinačnih testova za određivanje stabilnosti i mobilnosti lumbalne kralježnice te se javlja potreba za ponovnom sistematizacijom optimalnih testova za dijagnostiku funkcije lumbalne kralježnice kod zdravih i bolesnih pojedinaca. Cilj rada je opisati trenutno dostupne testove za dijagnostiku stanja lumbalne kralježnice te ponuditi protokol testiranja zdravih osoba kao i osoba sa lumbalnim bolnim sindromom. Iako se u dijagnosticiranju lumbalnog bolnog sindroma najčešće koriste provokacijski testovi kako bi odredili uzrok boli, u ovom radu je opisano tek nekoliko jer se prvenstveno htjelo prikazati testove izdržljivosti mišića te testove kontrole pokreta lumbalne kralježnice. U pretraživanju literature pronađen je velik broj testova od kojih je za potrebe ovog rada izdvojeno dvanaest, devet za osobe s lumbalnim bolnim sindromom te tri za zdrave osobe. Testovi kontrole pokreta temelje se na procjeni stabilnosti kralježnice tijekom izvođenja pokreta u sve tri ravnine sa kralježnicom u neutralnom položaju. Panjabijev neutralni položaj pokazao se kao bolji položaj za procjenu kontrole stabilnosti kralježnice jer najveći dio stabilizacije odrađuju aktivne strukture, tj. mišići, dok pasivne strukture u ovom položaju daju najmanji doprinos stabilnosti pokreta. Iako je rad podijeljen na testove za zdrave osobe i testove za osobe sa lumbalnim bolnim sindromom, treba naglasiti da se svi testovi za osobe s boli u donjem dijelu leđa mogu koristiti za zdrave osobe jer mogu koristiti u prevenciji lumbalnog bolnog sindroma. Zdrave osobe također mogu imati lošu kontrolu pokreta, stoga bi ispravljanje nekontroliranih obrazaca pokreta kod njih moglo pridonijeti smanjenju učestalosti bolova u lumbalnoj kralježnici. Isto tako, testove mišićne izdržljivosti mogu se koristiti i za osobe s boli u donjem dijelu leđa, ako im intenzitet boli to dozvoljava.

Abstract (english)

Low back pain is very common nowadays, so it is important to have a good diagnosis of the condition. There are many tests for determining the stability and mobility of the lumbar spine, and there is need to systematize optimal tests for the diagnosis of lumbar spine function in healthy individuals as well as individuals with low back pain. The aim of this paper is to describe the currently available tests for the diagnosis of the lumbar spine and to offer a testing protocol for healthy people as well as people with low back disorders. Although provocative tests are most commonly used in the diagnosis of low back pain to determine the cause of pain, only a few are given in this paper because this paper sought to present muscle endurance tests and lumbar spine movement control tests. Many tests were found in the literature search, of which 12 were extracted for the purposes of this paper, 9 for those with low back pain and 3 for healthy subjects. Movement control tests are based on the assessment of spine stability during the performance of movements in all three planes with the spine in a neutral position. Panjabi's neutral position has proven to be a better position for assessing spinal stability control since most of the stabilization is done by active structures (muscles), while passive structures (bones, ligaments) play a smaller part in movement stability in this position. Although the paper is divided into tests for healthy people and tests for people with low back pain, it is important to know that tests for people with low back pain can be used for healthy people as well because they can be used in the prevention of low back pain. Healthy individuals may also have poor movement control, so correcting uncontrolled movement patterns may contribute to reducing the incidence of low back pain. In that way, muscle endurance tests can also be used for people with low back pain if the pain intensity allows it.
