Abstract Ozljede adolescenata nedovoljno su istražen kineziološki i medicinski problem. Nepoznavanje specifičnosti ozljeđivanja lokomotornog sustava u znatnoj mjeri otežava mogućnost prevencije. Rad je dio Hrvatske longitudinalne studije tjelesne aktivnosti u adolescenciji CRO-PALS. Prospektivno istraživanje specifičnosti ozljeda lokomotornog sustava provedeno je s dvo-razinskim stratificiranjem slučajnog odabira. Od ukupno 2827 učenika upisanih u 1. razred 14 srednjih škola grada Zagreba, u drugom je stupnju uzorkovanja slučajnim odabirom izabrana polovina razrednih odjeljenja s 1408 ispitanika, od kojih je 903 (64 %) pristalo sudjelovati u istraživanju. Anonimni anketni upitnik ispunilo je 774 (54.8 %) ispitanika, 394 (50.8 %) učenika i 380 (49.2 %) učenica. Specifičnost ozljeđivanja analizirana je u odnosu na spol i stanje uhranjenosti, sudjelovanje ili ne sudjelovanje u natjecateljskom ili nenatjecateljskom sportu izvan škole, lokaciji i učestalosti jednostrukog, dvostrukog ili trostukog ozljeđivanja. Učenici i učenice srednjih škola grada Zagreba statistički su se značajno razlikovali prema sudjelovanju u natjecateljskom ili nenatjecateljskom sportu izvan škole (42.4 % naprema 25.3 %) (P=0.001), po učestalosti ozljeda lokomotornog sustava (33.5 % naprema 22.9 %) (P=0.001) i stanju uhranjenosti što podupire potrebu prevencije ozljeda ali i promicanje tjelesne aktivnosti na razini adolescentne populacije. Učestalost ozljeđivanja gležnja i koljena (19.8 % neprema 8.8 %), kuka (5.3 % naprema 1.2 %), ramena (8.4 % naprema 3.7 %), leđa (11 % naprema 6.7 %) i mekih tkiva (15.6 % naprema 7.2 %) značajno se razlikuje kod ispitanika koji sudjeluju i koji ne sudjeluju u izvanškolskim sportskim aktivnostima. Posebnu pozornost iziskuju mogućnosti prevencije ozljede gležnja i koljena budući da su to najučestalija lokacija ozljeđivanja s po 19.8 %). Specifičnosti prevencija ozljeda u adolescentoj dobi proizlaze iz rezultata koji pokazuju da je 48.1 % od 577 registriranih ozljeda lokomotornog sustava locirano na donjim ekstremitetima te da su u 43.6 % slučajeva ozljede nastale pri sudjelovanju u ekipnim sportovima s loptom (najčešće na nogometu, košarci, odbojci i rukometu). Ozlijeđeni i neozlijeđeni ispitanici značajno se razlikuju po učestalosti pothranjenosti (2.3 % naprema 6 %), učestalosti prekomjerne tjelesne mase (18.6 % naprema 13.0 %) i pretilosti (2.3 % naprema 3.1 %) što potvrđuje značaj tjelesne aktivnosti u prevenciji pretilosti. Rezultati provedenog istraživanja ukazuju na aktualan javno zdravstveni problem nedovoljne tjelesne aktivnosti, ali i na potrebu intenziviranja programa prevencija ozljeda u adolescentnoj dobi sukladno specifičnostima njihova nastanka.
Abstract (english) Injuries in adolescents are an insufficiently researched kinesiological and medical problem. Ignorance of the specific characteristics of injuries to the locomotor system significantly hinders the possibility of prevention. This prospective, longitudinal study is part of CRO-PALS research into the specific character of injuries to the locomotor system was conducted on a sample of 774 subjects, 394 (50.8%) male and 380 (49.2%) female high school students in the City of Zagreb,on the basis of a survey questionnaire and anthropometric measurements. The specific character of injuries was analysed in relation to gender and nutritional status, participation or non-participation in competitive or non-competitive sport outside of school, and the location and frequency of single, double or triple injuries. The male and female high school pupils in the City of Zagreb differed statistically significantly in terms of participation in competitive or non-competitive sport outside of school (42.4 % v. 25.3 %) (P 0.001), but not in terms of incidence of injuries to the locomotor system (33.5 % v. 22.9 %) (P=0.001), which supports the need for prevention of injuries but also the promotion of physical activity on the level of the entire adolescent population.. The incidence of injuries to ankles (19.8 % v. 8.8 %), knees (19.8 % v. 8.8 %), hips (5.3 % v. 1.2 %), back (11 % v. 6.7 %) and soft tissue (15.6 % v. 7.2 %) differed significantly between subjects who participate and those who do not participate in out-of-school sporting activities. The possibility of prevention of ankle and knees injuries demands particular attention, since this is the most frequent location of injury (19.8 %). The specific character of prevention of injuries in adolescence stems from the results which indicate that 48.1 % of the 577 registered injuries to the locomotor system were located in the lower extremities, and that in 43.6 % of cases the injuries occurred during participation in team sports involving a ball (most often football, basketball, volleyball and handball). The injured and non-injured subjects differed significantly in terms of the incidence of malnutrition (2.3 % v. 6.0 %), overweight (18.6 % v. 13.0 %) and obesity (2.3 % v. 3.1 %), which confirms the importance of physical activity in the prevention of obesity. The results of the research undertaken indicate the current public health problem of insufficient physical activity, but also the need to intensify programmes to prevent injuries in adolescence, in line with the specific character of their occurrence.
